The Germane & Wise approach to Collaborative Care Management leverages in-office psychiatric assistants acting as care managers to keep all team members in the know.
Trained in motivational interviewing, our psychiatric assistants can gather in-depth history on your patients’ metal health struggles and successes.
Our psychiatric consultant, (a psychiatrist or psychiatric nurse practitioner) reviews these reports as well as chart notes to formulate a working diagnosis and options for treatment.
While the PCP remains the head of the treatment team, the in-office psychiatric assistant serves as the hub for communication between all parties.
Calling your patients for regular follow-ups, the psychiatric assistant monitors your patients as they progress through treatment. This means:
patients always have a person they know they can ask for with questions about their treatment.
patients who are not improving are quickly identified and re-evaluated.
primary care doctors can be confident that they are working with true expertise and evidenced based interventions and are kept in the loop and up to date on a daily basis.
